I have a personal acquaintance with a an Emergency Room Pediatrician who works for a large children’s hospital in the Deep South. I asked for his thoughts on leading causes of death and below is his response.

It changes by age. But unintentional injury would be the most common cause. This would include all forms of unintentional injury. Most common unintentional injuries that cause death are motor vehicle accidents including ATV/golf cart. Others would be falls, and accidental gun deaths. As you get into the older age ranges/teens we see a much higher amount of intentional injuries suicide/homicide. So I guess I would say unintentional injury, suicide, homicide.
